- That email will have a link to our NEW PATIENT FORMS which must be filled out before your appointment.
- Please fill out patient paperwork prior to your appointment so Dr. Swanson can have time to review your history.
- If the paperwork is not completed, we will have to reschedule all new patient appointments.
